Estate Planning Documents

  • Healthcare Power of Attorney

Everyone needs a healthcare (aka medical) power of attorney, which tells healthcare professionals who can make medical decisions for you when you are unable.

  • Durable Power of Attorney

A durable (aka financial) power of attorney identifies who is authorized to handle your money and other assets when you cannot.

  • Last Will and Testament

A Will tells the Court who acts on your behalf after you die and how your assets are disbursed.

  • Trust

Like a Will, a Trust appoints someone to act on your behalf after your death and says how your assets are disbursed. Nothing needs to be filed in Court though, which saves your beneficiaries time and money, and protects everyone’s privacy.

  • Living Will

Similar to a healthcare power of attorney, this relieves your loved ones from having to make difficult end-of-life decisions for you in predesignated situations, providing medical professionals with your advance directive.

  • Trust Certificate

Provides basic trust information to those who need this information without divulging all the details regarding your assets.

  • Quit Claim Deed

Used in connection with a Trust to re-title your home in the name of your Trust.

  • Funding Instructions

Tells you what steps you need to take to place your assets into your trust (i.e. fund your trust).
